Instructions to Make an Appointment

Please be advised that each individual who registers for an appointment will need to do the following:

  1. Create an account and save the unique computer-generated Vaccination Code.

    Upon arrival at the OptumServe vaccination site, you will need to provide that Vaccination Code to the staff.

  2. You will register for both the 1st and 2nd dose of your Pfizer vaccine simultaneously. If there are no 2nd dose appointments available then you will be unable to book your 1st dose appointment.

  3. You will see a confirmation of your appointments with the dates, times, and location once the registration is finalized.
